    Sample answer outline

    1. Pick a real failure with stakes that was your responsibility. Not a disguised success like "I cared too much".
    2. Set it up in two sentences, then own your part plainly: what you did or did not do.
    3. Spend most of the answer on what you did next: how you repaired what you could, who you told, what you changed.
    4. Prove the lesson stuck with a later moment where you handled a similar situation differently.

    What makes it work

    • Ownership without self-punishment reads as maturity.
    • The later example turns a lesson into evidence.

    Avoid

    • Blaming teammates or circumstances.
    • A failure so small it shows nothing, or so large it raises red flags you can't resolve.

    Sample answer outline

    1. Set the scene fast: the team, the goal, and what was going wrong.
    2. Diagnose before acting: what you found out about why it wasn't working (unclear roles, conflict, workload).
    3. Describe your specific actions: a conversation, a new plan, sharing the load. Use "I", not only "we".
    4. Give the outcome honestly, including what didn't fully work.
    5. Close with what you do differently on teams now.

    What makes it work

    • Diagnosis before action shows judgment, not just energy.
    • Honest results are more credible than a perfect turnaround.

    Avoid

    • Blaming one teammate for everything.
    • Saying you just did all the work yourself.

    Sample answer outline

    1. Choose a disagreement about substance (a plan, a process, safety), not a personality clash.
    2. Show you checked yourself first: did you have the full picture?
    3. Describe raising it respectfully and privately, with facts and a suggestion.
    4. Give the result, and show you accepted the decision or escalated only when the stakes justified it.
    5. Reflect on what you learned about speaking up.

    What makes it work

    • It shows you can disagree without damaging the relationship.
    • Proportion: escalating only when it truly matters.

    Avoid

    • A story where you were simply right and they were simply wrong.
