    Sample answer outline

    1. Pick a real failure with stakes that was your responsibility. Not a disguised success like "I cared too much".
    2. Set it up in two sentences, then own your part plainly: what you did or did not do.
    3. Spend most of the answer on what you did next: how you repaired what you could, who you told, what you changed.
    4. Prove the lesson stuck with a later moment where you handled a similar situation differently.

    What makes it work

    • Ownership without self-punishment reads as maturity.
    • The later example turns a lesson into evidence.

    Avoid

    • Blaming teammates or circumstances.
    • A failure so small it shows nothing, or so large it raises red flags you can't resolve.

    Sample answer outline

    1. Set the scene fast: the team, the goal, and what was going wrong.
    2. Diagnose before acting: what you found out about why it wasn't working (unclear roles, conflict, workload).
    3. Describe your specific actions: a conversation, a new plan, sharing the load. Use "I", not only "we".
    4. Give the outcome honestly, including what didn't fully work.
    5. Close with what you do differently on teams now.

    What makes it work

    • Diagnosis before action shows judgment, not just energy.
    • Honest results are more credible than a perfect turnaround.

    Avoid

    • Blaming one teammate for everything.
    • Saying you just did all the work yourself.

    1. Don't look at it, and say so.
    2. Find out what you can without spreading it: is it really the exam, and how widely is it shared?
    3. Encourage the classmate to stop and come forward.
    4. Tell the course director either way. The exam's fairness affects everyone, and future patients depend on honest assessment.
    5. Stay fair: you're reporting facts, not judging the person.

    What makes it work

    • Links academic honesty to patient trust, the real stakes.
