You never showed me
You train new volunteers at an animal shelter.
Last week you trained a new volunteer, Hye-jin, including how to secure the kennel doors. Today a dog got out of a kennel she had closed and ran into the parking lot before staff caught it. When the shelter manager asks what happened, Hye-jin says you never showed her the second latch. You are sure you did, but you trained four people that day.
