Preparing for the Indiana University of Pennsylvania College of Osteopathic Medicine interview

IUPCOM's 2-on-1 interview for its founding class: format per AACOM, rural Pennsylvania mission, 2026-27 dates and practice questions we wrote.

IUPCOM DO interview guide title card: format, timeline and practice questions

The IUPCOM interview format

Indiana University of Pennsylvania's new College of Osteopathic Medicine (IUPCOM) has not described its interview on its public program page, so most of what is public comes from its AACOM Choose DO Explorer profile, which AACOM says is school-submitted. That profile says qualified applicants "will be selected for an interview with the admissions team," that interviews are conducted in person (virtual interviews can be arranged "due to extenuating circumstances"), and that they use a "2-on-1 format," meaning two faculty members and one applicant. AACOM lists the format as traditional and says the admissions staff "strongly prefers" that candidates interview in person at the Indiana, Pennsylvania campus. IUPCOM does not publish the length or whether the file is open or closed.

What IUPCOM is looking for

IUPCOM's mission is "to increase the number of community physicians throughout rural and underserved Pennsylvania while creating a national model for rural healthcare," through an education that is "evidence-based, inspiring, compassion-focused, holistic, community-centered, and affordable." AACOM says the college is "looking for qualified applicants who have a passion for serving in rural and medically underserved areas of Pennsylvania." The program page says the college is focused on recruiting from Pennsylvania's State System of Higher Education universities, and that its curriculum has a behavioral and mental health emphasis, including a core rotation, because rural physicians are often the first point of contact for many problems.

What the school namesHow it might show up in your answers (our suggestion)
Rural and underserved PennsylvaniaA specific tie to a rural place, and why you would stay
Community-centered careExperience working with a community rather than only for it
Behavioral and mental healthWhat you have seen about mental health access, and what you would want to learn
Holistic, whole-person careA time you treated the person, not only the problem
Affordability and public missionWhy a public school's mission matters to you

Timeline for the current cycle

  • AACOMAS opened May 4, 2026 and the primary deadline is April 1, 2027. AACOM says to apply before December 1, 2026 for best consideration, with applications considered until the deadline.
  • There is an Early Decision track for Pennsylvania residents (AACOM): application due August 1, 2026, supplemental materials October 1 and notification November 1, 2026. That date has passed for this cycle.
  • Requirements on IUP's page: a C or better in biology 8, physics 8 (or 4 plus 4 of advanced math), general chemistry 8, organic chemistry 8 (biochemistry may substitute for 4), English 6 and behavioral sciences 6. A competitive application has a 3.4 overall and 3.0 science GPA, with a minimum MCAT of 490 taken within three years of matriculation. IUP says higher scores are generally needed to be competitive.
  • AACOM says no physician letter is required and that only in-person shadowing is accepted.
  • Tuition for the first year is $43,200 for Pennsylvania residents and $65,000 for non-residents, per IUP's page.
  • IUP says it has not yet received federal authorization to issue Form I-20 and that international students may not be able to enroll for the July 2027 start. Confirm with admissions if this applies to you.

An inaugural class: what to know

IUPCOM earned COCA pre-accreditation in May 2026 and plans to welcome its first students in fall 2027, so applicants this cycle are the founding class. IUP says it would be the only college of osteopathic medicine at a public university in Pennsylvania, and that it has secured more than 270 percent of the clinical training placements needed for accreditation. Its program page also describes early clinical experiences, simulation, standardized patients and a required research project and scholarly activity rotation. If you are asked why you would join a founding class, show that you understand the trade-off: you give up a track record and gain the chance to help shape a program. Say what you would do to help it succeed.

How to prepare

  1. Plan for two interviewers and one of you. Practice holding eye contact with two people, splitting your attention fairly and answering whichever one asked.
  2. Anchor your rural story. Have two or three concrete reasons you want to serve rural or underserved Pennsylvania, including what you would miss and what you would do about it.
  3. Know the osteopathic basics. Be able to describe the four tenets in your own words and what you have seen DOs do.
  4. Prepare a mental health answer. The curriculum names it as a focus, so know why it matters in rural practice.
  5. Be ready for either setting. If you go in person, plan the trip. If virtual, test your connection and room.

Practice questions (written by us, not IUP)

IUPCOM does not publish its interview questions. We wrote these around its stated mission. They are for practice and do not predict what you will be asked.

  1. Walk me through why you want to become an osteopathic physician.
  2. Why IUPCOM, and why join a founding class?
  3. What draws you to practice in rural or underserved Pennsylvania?
  4. Tell me about a time you worked in or with a small community. What did you learn?
  5. A rural patient with depression has no therapist within an hour. What do you try?
  6. Tell me about a time you were the only person with a certain perspective in the room.
  7. How do you handle a mistake that affects someone else?
  8. What does holistic care mean to you beyond treating the whole body?
  9. A patient cannot afford a follow-up visit. What do you do?
  10. Describe a time you took on a role no one had done before.
  11. What would you do if the first year of a new program did not go as planned?
  12. What question were you hoping we would ask?

A 30-minute practice run

  1. Recruit two people to play the two interviewers and ask them to read your application first.
  2. Have them pick eight questions from the list above and alternate asking, with a follow-up each.
  3. Keep answers to about two minutes and run it in the setting you expect, in person or on Zoom.
  4. Record it. Watch two answers and check whether you named a specific example, spoke to both interviewers and answered what was asked.
  5. Ask what they would still want to know, and add that detail next time.
