The Casper Test, Explained (2026-27)

The Casper test is an online situational judgment test from Acuity Insights that many medical, PA, dental, nursing, pharmacy and vet programs require. You answer 11 scenarios (4 by video, then 7 typed), each with 2 questions, in about 65 to 85 minutes. Programs get your score, and you get a quartile.

What is the Casper test?

Casper is a test of how you think through everyday situations with other people: a disagreement in a group, a friend who asks for an honest opinion, a rule that seems unfair. It is not a knowledge test, so there are no facts to learn and no right answer to memorize.

Acuity names nine aspects that its scenarios look at. Programs use the result next to grades and interviews to judge the people skills that grades don't show.

  • Collaboration
  • Communication
  • Empathy
  • Ethics
  • Fairness
  • Motivation
  • Problem solving
  • Resilience
  • Self-awareness

Casper test format (2026-27)

Casper has 11 scenarios in two sections: 4 video-response scenarios first, then 7 typed-response. Each scenario has 2 questions. Many older guides and school pages still describe the previous version, with 14 scenarios, 3 questions each and 90 to 110 minutes, so go by the format below.

Casper video and typed sections

BreaksUp to 10 minutes after the video sectionUp to 5 minutes after the first 4 typed scenarios

The clock keeps running during breaks. Video answers save when the minute ends, and typed answers save and move on when time runs out. For timing tips by section, see how to prepare for Casper.

How is the Casper test scored?

Trained human raters score every answer, and a different rater reads each scenario, so one first impression can't color the whole test. Your ratings are compared with other applicants on the same test type. Programs receive an overall score, and you receive a quartile.

Raters
Anonymous answers, no names or photos. Each rater scores one scenario for many applicants, and scores each question separately.
What raters ignore
Spelling, grammar and bullet points. They score your thinking, judgment and how clearly you explain it.
Quartiles
1st is the 0 to 24th percentile, 2nd the 25th to 49th, 3rd the 50th to 74th and 4th the 75th to 100th.
When results arrive
Programs about 2 to 3 weeks after the test. You get your quartile by email about 4 to 5 weeks after it.
Re-scoring
Not offered, and there is no feedback on individual answers.

Schools weigh Casper very differently. McMaster counts it for 32% of its interview-invitation formula and McGill for 20%. Manitoba ends eligibility below a set floor, and Boston University's PA program asks for the 3rd quartile. Many US schools only ask that you complete it. More in how Casper is scored.

How to prepare for the Casper test

You can't study facts for Casper, but you can get used to the timers and to giving a fair, complete answer quickly. One to three weeks is enough for most people.

  1. 1

    Learn the format

Know the two sections, the countdown and the shared 3.5-minute typed clock before you practice.

  2. 2

    Take Acuity's free practice test

It shows the official screens and timers. It isn't scored and doesn't match the real test's difficulty.

  3. 3

Practice on real timers, out loud and typed

Answer on camera for the video section and type both answers inside 3.5 minutes. Try a free sample scene or full Casper test prep.

  4. 4

    Review against the aspects

After each set, check whether you named everyone affected, asked what you didn't know, and said what you would do and why.

  5. 5

    Run the system check

Acuity requires it within 6 hours before your test, on the same computer, connection and room.

A simple approach for most scenarios: acknowledge everyone involved, find out the facts, then choose a fair option and explain why. More in how to prepare for Casper and our Casper sample questions.

What can you have during the Casper test?

You take Casper alone, at home or anywhere quiet, with your webcam on the whole time. Acuity staff may watch a session remotely.

Allowed
Pen and paper, a physical dictionary, headphones, an external keyboard and a drink.
Not allowed
Notes prepared beforehand, a phone, a smartwatch or voice assistants, unless approved as an accommodation.
Before you start
Restart your computer, close other apps, turn off extensions such as Grammarly and any VPN, and log in about 30 minutes early.

Casper vs PREview vs Duet

Casper
Acuity Insights' open-response test. You write and record your own answers, and raters score them.
AAMC PREview
The AAMC's own situational judgment exam, used by some US MD and DO schools. You rate how effective given responses are instead of writing your own, and it doesn't replace Casper where a school asks for Casper. Casper vs PREview
Duet
Acuity's site no longer lists Duet. Check your school's page if it asks for it.

Frequently Asked Questions

Quick answers about the Casper test.

No. There is no pass mark. Programs get an overall score that compares you with other applicants, and you get a quartile, 1st to 4th. A few programs set their own minimum, so check each school.
A 3rd or 4th quartile means you scored above at least half of the applicants who took the same test type. Most schools treat Casper as one part of the file, and only a few set a cutoff.
About 65 to 85 minutes: 11 scenarios (4 video, then 7 typed) with 2 questions each, plus two optional breaks. Video answers get 1 minute each, and each typed scenario gets 3.5 minutes for both answers.
Not in the same cycle for the same test type. Acuity only reconsiders a retake for a verified technical problem reported within one week. A result counts for one admissions cycle, so reapplying means a new test.
There is nothing to memorize, but you can practice. Knowing the timers, answering out loud on camera and typing two answers inside 3.5 minutes all get easier with timed practice.
No. Acuity tells raters to ignore spelling and grammar, and bullet points or an unfinished last sentence are fine. A typo that changes your meaning can still confuse the rater.
No. You need a desktop or laptop with Chrome or Firefox, a working webcam and microphone, and a steady internet connection. Phones and tablets aren't supported.
One admissions cycle, for the test type you took. A 2026-27 result can't be reused for the 2027-28 cycle.
Programs get your result about 2 to 3 weeks after the test. You get your quartile by email about 4 to 5 weeks after it, in your Acuity account.
$85 for US medicine, with 9 programs included and $20 for each extra one. Other US tests are $50, and Canadian tests are CA$50. Rescheduling within 30 days of the test costs $20.
The next US Medicine (MD and DO) sittings are Oct 27, Nov 12 and Dec 3, Eastern time (Acuity's list, checked Oct 23). Book at least 3 days ahead, and check which dates your programs accept.
