All 33 US MD and DO schools on Acuity's 2026-27 Casper list: what each school's own page says, last test dates, deadlines and fees.

Casper is the situational judgment test run by Acuity Insights, and each school decides whether to ask for it. As of October 6, 2026, Acuity's Dates and Fees page lists 33 US medical schools on the Medicine test (CSP-10111) for the 2026-27 cycle: 29 MD and 4 DO. This guide goes one step further than that list. For each school it shows what the school's own admissions page says, because the two don't always match, plus the last test date and score deadline Acuity lists.

The short version: 25 of the 33 clearly ask for Casper (21 MD and 4 DO), 2 accept either Casper or the AAMC PREview exam, 1 says applicants are expected to take it, and 5 are on Acuity's list but say on their own pages that Casper is optional, encouraged only, not used or not mentioned.

On Acuity's list, but not required by the school's own page (5): Central Michigan (optional), Rush (no longer required or recommended for Fall 2027), Tulane (encouraged, interviews with or without it), LSU New Orleans (its requirements and deadlines pages don't mention Casper) and Drexel (not listed for the MD program, only for BA/BS+MD).

That last group is why we checked every school's own page. Acuity's list tells you which test you would register for. The school's page tells you whether you need it at all. Where they disagree, go by the school, and email admissions if its page is unclear.

You need an ID number to register your schools: an AAMC ID or AMCAS ID for MD schools, a TMDSAS number for several Texas schools, and an AACOMAS ID for the DO schools. Texas Tech El Paso and Sam Houston State ask only for the TMDSAS number. Baylor, UT Tyler, McGovern, UT Health San Antonio, UTMB and UT Southwestern ask for both an AAMC or AMCAS ID and a TMDSAS number. Touro also accepts its own student IDs.

A school that is not in these tables is not on Acuity's 2026-27 list for the US Medicine test. Some of those use the AAMC PREview exam instead, and some use neither, so check the school's own admissions page.

Casper test dates for 2026-27

Acuity lists US Medicine test dates from March 19, 2026 through February 9, 2027, and each school accepts the dates up to its own last test date. As of October 6, the dates still open for most schools are October 20, October 27, November 12 and December 3, 2026, then January 5 and February 9, 2027. Acuity shows start times in your own time zone, so check the exact time on its page.

A few schools set their own cutoff, and it can differ from Acuity's. Boston University says the latest Casper date it accepts is October 28, 2026. UT Southwestern wants scores distributed by November 1. Quinnipiac Netter lists December 1 where Acuity lists November 30. New York Medical College wants all supplemental steps done by January 31. When two dates differ, use the earlier one.

How to pick your Casper date

  • Work backward from your earliest school. Take the earliest last test date among your schools and plan to test well before it. Acuity says results go to programs about 2 to 3 weeks after your test, so testing on a school's last date leaves no room for a problem (Acuity Help Centre).
  • Test early if your schools review in order. Several schools say they won't consider your file until the score arrives: Baylor, ETSU Quillen, Texas Tech El Paso, UTMB and Zucker. New York Medical College lets you keep moving through review while your test is registered or your score is pending, but it says an earlier complete file is reviewed earlier.
  • You get one try. Acuity does not let you take the same test type more than once per admissions cycle, so there is no retake to improve a score. Schedule it when you are ready, not just when a slot is open.
  • Add schools any time before their deadline. You can add a program to your distribution list before or after you take the test, as long as its distribution deadline hasn't passed. If you apply to a school later, you do not need a second test.
  • A past score doesn't carry over. Scores are valid for one admissions cycle. Re-applicants retake it, and a 2025-26 score doesn't count for 2026-27.

Registration, fees and what else to know

  • Fee: $85 for the Medicine test, which includes sending your score to 9 schools of your choice, then $20 for each additional school. Fees are final and non-refundable, and a fee applies if you reschedule within 30 days before your test or after it. Every school on the list accepts the Fee Assistance Program (FAP).
  • Account: Create an Acuity account with a government-issued photo ID, and use the same name and email as on your application.
  • Timing: Register at least three days before the test to leave time for ID checks and payment. If you need accommodations, submit your forms four weeks before the test.
  • Duet: Acuity charges nothing extra for Duet if a program requires it. Boston University and Temple both say they don't use Duet, so check your school's page.
  • Test type: Your score only works for the test type you took. The Medicine test covers MD and DO. A score for a different type, such as a PA or dental test, or for a program in another country, doesn't transfer.

What the test is like

On Acuity's applicant page, the current test is 11 scenarios in two sections: 4 video-response scenarios, then 7 typed-response scenarios, with 2 questions each. You get 1 minute per video answer and 3.5 minutes to type both answers for a typed scenario, and the whole test takes about 65 to 85 minutes with two optional breaks (About Casper). Which medical schools require Casper?

For 2026-27, 25 schools on Acuity's US Medicine list say on their own pages that Casper is required: 21 MD and 4 DO. They include Baylor, Boston University, Medical College of Wisconsin, New York Medical College, Temple Katz, Wake Forest and VCU. The full table above shows each one with its last test date.

Does every medical school require Casper?

No. Acuity lists 29 MD and 4 DO programs for the US Medicine test, which is a small share of US medical schools. Many schools don't use it, and some use the AAMC PREview exam instead. Michigan State College of Human Medicine and Rutgers Robert Wood Johnson accept either.

Is Casper required for Texas medical schools?

Eight Texas MD schools are on Acuity's list: Baylor, TCU Burnett, Texas Tech El Paso, UT Tyler, UTHealth Houston McGovern, UT Health San Antonio, UTMB and UT Southwestern. Sam Houston State is the Texas DO school on it. Their own pages ask for Casper. Most of them also ask for your TMDSAS number when you register.

Is Casper required for DO schools?

Four DO schools are on the 2026-27 list: Sam Houston State, Touro NY/MT, Northern Colorado and William Carey. All four say it is required. Most other DO schools don't list it.

Do I need to retake Casper if I took it last year?

Yes. Acuity says scores are valid for a single admissions cycle, and schools such as New York Medical College and Stony Brook say re-applicants must retake it.

How much does Casper cost?

$85 for the US Medicine test, including your score going to 9 schools, then $20 for each extra school. Fees are non-refundable, and Acuity lists the Fee Assistance Program as accepted for every school here.

What if my school is on Acuity's list but says Casper is optional?

Follow the school. Central Michigan, Rush and Tulane all say on their own pages that it isn't required. If a page is silent, as at LSU New Orleans, ask admissions before you pay for a test you may not need.
