How Casper Is Scored: Quartiles and What a Good Score Means
How Casper is scored, from Acuity's own pages: human raters, how quartiles work, what programs see, when results arrive, and what a first-quartile result means.

Casper results are the part of the test people understand least, because applicants never see a score. This guide covers only what Acuity Insights says publicly about how Casper is rated, what the quartile you receive means, and when programs and you get results. Where we add our own view, we say so. For the test itself and 8 practice scenarios, see our Casper format and practice guide.
Who scores Casper
Trained human raters score every Casper test. Acuity says the test is not graded by AI and that raters are selected to reflect the population of the country where your program is located (Acuity help article). Raters complete a training program, pass a calibration step before they rate live responses, and are recertified each year. Acuity also blocks raters who move through responses too fast and monitors how consistently raters score the same responses (rater training).
How your responses are rated
From Acuity's page on how Casper is scored:
- Each scenario has two questions. The same rater sees both, with the scenario in front of them, but scores each question on its own.
- Your responses are anonymized. Raters do not see your name, age or photo, only your typed or video answers.
- A rater is given many different applicants' responses to one scenario. No rater scores you more than once, so each of your scenarios is judged by a different person.
- Raters are told to focus on substance and quality, not grammar or spelling. They look at your thought process, judgment and communication.
- The ratings are averaged and then standardized to show your standing relative to the applicant pool. Your final score is built from all the individual question scores, typed and video together.
Two more points from Acuity. Casper is not pass or fail and does not use a fixed rubric; it compares you with applicants to similar programs in the same country and language (before, during and after Casper). And a test cannot be re-scored, because no single rater sees more than one set of your responses (re-scoring).
What a quartile is
Applicants receive a quartile, not a number. Casper scores are grouped into four quartiles that compare you with other applicants on the same test type. Acuity says the quartiles are determined by how strong your responses are relative to others who answered the same scenario. Acuity's advisor information describes each quartile as 25% of applicants, so the first covers the 0 to 24th percentile and the fourth the 75th to 100th (Acuity advisor page).
| Quartile | Percentile range | Meaning |
|---|---|---|
| First | 0 to 24 | Other applicants' responses were rated stronger |
| Second | 25 to 49 | Below the middle of the pool |
| Third | 50 to 74 | Above the middle of the pool |
| Fourth | 75 to 100 | Among the highest-rated responses |
The "Meaning" column is our plain restatement of the ranking idea. Acuity's own wording for the first quartile is that other applicants had stronger responses, which does not mean you failed (your quartile score). Acuity adds that the Casper pool includes many high-caliber applicants.
What programs see, and what you see
- Programs get your result about 2 to 3 weeks after the test, sent straight to the programs on your distribution list. Programs you add later get it within one business day. You get no email about this; check your account for "Completed - Results delivered" (when results are sent).
- You get a quartile, usually 4 to 5 weeks after your test, with an email and a "View quartile score" link in your account. For CaRMS applicants in Canada, Acuity says the quartile is available by December 15.
- You do not get the underlying score. Acuity says it is one combined number with no breakdown by topic, so sharing it would not show you where you did well or badly (will I receive my score).
How programs use Casper
Each program decides. Acuity lists three uses: filtering applicants early, deciding who is invited to interview, and ranking at the end (how programs use Casper). It adds that the same result can be more competitive at one program than another, depending on the pool, and that most programs consider all quartiles and treat Casper as one data point among many.
The schools themselves vary. Baylor says a Casper score is needed to be considered for an interview. Other schools ask for it later: UT Health San Antonio's page says Casper is not used in screening and is required by the interview date. See each school's row in our list of medical schools that require Casper, which links to the school's own page.
Why some applicants land in a higher quartile
Acuity names five things that may separate higher quartiles from the first: effort (explaining your position fully and using the full time), empathy (weighing all perspectives meaningfully), impartiality (respect for others' experiences and needs), communication (articulating ideas well) and familiarity with the format (Acuity quartile article). It says typos and incomplete sentences do not affect your score, and that it is normal not to finish typing a thought (spelling and grammar).
Will you score the same next time
Acuity says Casper has relatively high test-retest reliability, so applicants can expect similar results across tests, though some people see a different quartile between tests (Acuity quartile article). It also says surface strategies, such as trying to game the test or "fake good", will not improve your score, and that familiarity with the format helps slightly (improving). Our view, which is not Acuity's: that makes the practical advice simple. Learn the format, answer the actual question, and show how you weigh the people involved.
If you want to practice, Acuity's own free 11-scenario practice test is the place to start. For extra timed practice with feedback, Confetto has an independent Casper Prep with a free set of video and typed scenarios (current pricing is on that page). FAQ
What is a good Casper score?
Acuity doesn't define one. A higher quartile is a stronger relative result, and each program decides what it needs. Most programs consider all quartiles, so a first-quartile result should not stop you applying.
Can I see my Casper score?
You see a quartile only, around 4 to 5 weeks after testing. You do not see the underlying score.
Is Casper graded by AI?
No. Acuity says trained human raters score it, with each scenario rated by a different rater.
Do spelling mistakes lower my Casper score?
Acuity says raters are instructed to focus on substance, not grammar or spelling.
Can I request a re-score?
No. Acuity says Casper tests cannot be re-scored.
How long are Casper results valid?
One admissions cycle and one test type. A new cycle needs a new test. Booking details are in our test dates and registration guide.
